What do I need to bring?

You only need to bring a photo ID and a list of medications!

*It is not required that you wear a mask during the entirety of your appointment. Our staff may be wearing one and you are invited to wear one if you would like!

What will the appointment be like?

It is only 4 easy steps!

1. Check in with your photo ID with the receptionist.
2. You will be escorted to an exam room where your vital signs including height, weight and an eye exam will be performed.
3. An overview of your entire medical history and allegations will be completed.
4. The physician will complete a physical exam.

All done!
